Health Care Management

On enrolling your child, you will be asked to provide your child’s health information to help us meet your child’s health needs.  This will include:

  • providing your child’s Australian Immunisation Register (AIR) immunisation history statement. Further information on obtaining the necessary Immunisation History Statement can be found under ‘IMMUNISATION’ in our Parent Information Handbook.
  • completing a Health Care Summary form with details about health care needs and information to use in a medical emergency.
  • completing a Management and Emergency Response Plan where the Health Care Summary indicates your child needs support and/or medication at school.

For the safety and care of ALL students, all medications (prescribed and over-the-counter medications) are to be stored and managed by school staff only.

Medications can only be administered by school staff if the necessary documentation has been completed by parents/carers.  This applies to both prescribed and non-prescribed medications.

Short-term Medication

If your child requires administration of short-term medication (up to two weeks) such as antibiotics or pain relief, we require your written authority to do so. This authority must be provided by completing this Administration of Medication form and the required medication to the school office.

Long-term Medication

If your child has an ongoing condition that may require the school to administer medication to your child at school, you will need to complete the relevant Management/Emergency Response Plan for your child’s particular health need. This process includes:

  • providing us with clear information about which foods, insect bites, stings or other allergens trigger an allergic reaction in your child.
  • providing us with any individualised Action Plan eg Anaphylaxis Plan, Minor to Moderate Allergy Plan, Asthma Plan, Other, as well as any preventative strategies that can be put in place
  • supplying us with the necessary medication, eg EpiPen, antihistamines, asthma medication etc, ensuring it is clearly labelled (check and record expiry dates of all medications and replace prior to expiry).

Parents are required to inform the school immediately of any changes to these details.

Student Health Care Management

Food Allergies

Our school does its best to be an allergy-aware school. Staff have had training in severe allergy management, and we have implemented strategies to help keep students at risk of anaphylaxis as safe as we can, with key staff trained in First Aid.

Please talk to your child’s teacher to discuss sending food to school for birthday celebrations.

School Nurse

We do not have a school nurse on site. This service is provided by the Health Department to undertake health assessments. Visits are arranged by the school.

Dental

The School Dental Service (SDS) provides free dental care to students from the age of 5 (to 16) who attend a Department of Education recognised school. The Mount Henry Dental Clinic in Como is the SDS for Applecross Primary School. There are no charges for inspection and simple level of operations, however we must have a consent form from parents before children can participate. Parents are responsible for transporting children to and from the clinic.

We acknowledge the traditional custodians of the land and waters on which we gather. Applecross Primary School has stood on these lands since 1905. Our Scar trees form a part of our rich history, each telling a story of life before our school began. These are the lands of the Whadjuk Noongar people, always was, always will be. We pay our respects to their Elders past and present.
